Here we go as this assumes it could be out for a while:

(2) and (5) in the Bronx both run between East 180th and their respective northern terminals (241st Street and Dyre Avenue) respectively).  Some trains are extended to Freeman Street southbound.

(2) also runs from 148th Street-Lenox Terminal to Flatbush Avenue.  Some (2) trains in Manhattan terminate at 137th Street on the (1) and return from there.

(3) service runs normal and is unaffected.

(4) shuttle train runs 161-Yankee Stadium to Woodlawn

Regular (4) service runs between 125th Street and Utica Avenue at all times. 
Some (4) service to 125th Street is extended northbound to either 3rd Avenue or East 177th-Parkchester on the (6) 
(5) runs in the same manner as the (4) in Manhattan and Brooklyn (though in Brooklyn to Flatbush Avenue) with some trains extended along the (6) 

All (6) service is local in The Bronx due to some (4) and (5) service running express there.

Shuttle buses run from 125th Street/Lexington Avenue to 161st Street on the (4) and East 180th Street on the (5).

Shuttle buses run from Central Park South/Lenox Avenue on the (2) to East 180th Street.

(B) runs at all times between Brighton Beach and Bedford Park Boulevard

(D) runs rush-hour level service AM Rush to Midnight.

Then 60th would almost certainly also be flooded.  Assuming that, and this pattern having to be in place for an extended period:

No service directly between Queens and Manhattan EXCEPT on the (7).   Also no service to Roosevelt Island, which is replaced by shuttle buses between Roosevelt Avenue and there (with a stop at 21st-Queensbridge).

Shuttle buses run between Roosevelt Avenue in Queens and 50th Street-8th Avenue in Manhattan. stopping at 21st-Queensbridge, Queens Plaza, Queensboro Plaza, 63rd Street-Lexington Avenue, 60th Street-Lexington Avenue 57th Street-6th Avenue, 57th Street-7th Avenue and 50th Street-8th Avenue.

(E) runs LIMITED service between Chambers-WTC and Lexington Avenue-53rd Street.

(F) is split as follows:
Trains to/from 179th Street run at all times via the Crosstown Line between Roosevelt Avenue and Bergen Street.
Trains to/from Manhattan run regular route to 57th Street/6th Avenue and terminate there.
Some (F) trains also run 21st-Queensbridge to 179th Street.

(G) is extended at all times to Jamaica Center to replace the (E) in Queens.

(M) runs regular route at all times to Lexington Avenue-53rd Street, where it terminates 24/7 during flood work.

(N) runs in two sections:
Regular route to 57th Street-7th Avenue and with the (Q) to 96th Street-2nd Avenue.
Queensboro Plaza to Ditmars Boulevard.

(R) runs regular route to Lexington Avenue-60th Street, where it terminates.

(W) is suspended.

@Wallyhorse is not reading carefully. 60th St tunnel is not closed.

(B)(W) : Service suspended.   

(A)    Runs local between 59th St and Canal St

(E) : No trains along 8th Av between Queens Plaza and World Trade Center. Trains run via  (R) between Queens Plaza and Whitehall St.

(F) : Trains run in 2 sections. Train will run local in Queens. 

1. Between Jamaica-179th St and 21st St-Queensbridge.

2. Between Lex Av-63rd St and Coney Island-Stilwell Av.

(M) :  Trains rerouted over to  (B) between 47th-50th Sts and 145th Sts.

Free loop shuttle buses operates from Queensboro Plaza, Queens Plaza, Court Sq, 21st St-Queensbridge, Roosevelt Island, 21st St-Queensbridge, than return to Queensboro Plaza.

Additional bus service added on Q32 bus between 74th St and Grand Central.

Additional Q102 bus service will be provided.

(Q) : No trains between 96th St and Lex Av-63rd St. 

Free shuttle buses provided along 1st-2nd Avs between Lex Av-63rd St and 96th St-2nd Av, stopping at Lex Av/60th-59th Sts station.

Shuttle bus routing: Lex Av, 57th St, 1st Av, 99th St, 2nd Av, 66th St, Lexington Av.

In this hypothetical situation (a flood) if the subway tubes both north and south of the 60th St Tunnel are flooded, it is okay to presume that the 60th St Tunnel would be flooded too. The response of @Wallyhorse was more effective than the suggestion of just cramming the (E), with it's 12+ tph during rush hours, in the tunnel with the (N) and (R)

Empire State Building collasped onto 34th St-Herald Square and 34th St-Penn Stations, shutting down Broadway line between 23rd St between Times Sq, 6th Av between 23rd St and 42nd Sts, 7th-8th Avs between 23rd St and 42nd Sts, along with 6th Av PATH stations service suspended.

Affected (1)(2)(3)(A)(C)(E)(B)(D)(F)(M)(N)(Q)(R)(W) PATH, LIRR, (NJT) Amtrak.

Also streets between 30th and 42nd sts are closed, and between 9th Av and Mad Avs.

Bonus: Reroute buses as well.

There's no way to physically do that using the existing track layout. The whole point of this thread is to reroute trains using existing tracks. 

Furthermore, if the Empire State Building collapsed vertically (hypothetically), it wouldn't even get to 7th Avenue, come on now.

Oh, I did not know that the (B)(D) did not connect to the (E).

Scenario: Sunnyside Yards and Queensboro Plaza get hit by a meteor. No trains can run through this area. Also, a (G) train lost control and hit the support beams north of Court Sq, so no service on the (M)(R) at Queens Plaza (Express tracks not affected). While this is happening, the 63 St Tunnels flood, and Lex-63 Station is also impassable. However, 53 and 60 St tunnels are still open.

REROUTE AWAY!

Scenario: Earthquake in Harlem (on the Harlem Fault Line). All stations along 125 St are closed, along with 110 St (2)(3), 110 St (6), and as a precaution, The Astoria Line (N)(W). Also as a precaution, MNR cannot run in Manhattan, and the 63 St Tunnels are closed.

 

Reroute: (1)(2)(3)(A)(B)(C)(D)(4)(5)(6)(N)(W)(F), MNR, and Amtrak (Empire Corridor).

AM Rush 7:15 AM

A bomb threat at Jackson Heights/74 Street requires immediate evacuation of the area. No trains are allowed to pass through this stop.

Meanwhile, a water mainbrake at Grand Street has the Chrystie St connection backed up

There are unauthorized people on all tracks of the Manhattan Bridge because they mad about something so there a crowd of people littered all over the tracks. Police get there and a person warns a cop that he'll touch the 3rd rail, so there is a standstill on the bridge. Trains are unable to use the Manhattan Bridge. 

Then a unruly passenger pulls the emergency brake on the (7) in the between Grand Central and Vernon Blvd.

Also a Person jumped I front of the (1) at 191 St Manhattan Bound.

In addition, major signal problems are occurring on the (2) and (5)  and halts Service south of 3 Av 149 st.

Then a Manhattan Bound (J) catches fire at Marcy Av.

Then a (A) train derails as it leaves Grant Avenue damaging Queens and Manhattan bound tracks.

Manhattan bound (F) Trains are stuck at Smith 9 street because of debris on the track. The Express tracks are undergoing repairs after a cracked rail, and no Express trains are allowed to run.

Sadly, A work train broke down inside of the city hall loop affecting (6) trains

Then in the Bronx, A fight breaks out at Bedford Park Blvd on a Manhattan bound (4) which requires police to be called.

Then a shoot out is happening in Coney Island terminal with rival gangs. All platforms are not to be used in Service.

And for the Cherry on top. We have a major collision in Times Square were a (N) rear ended a (Q) and a fire engulfs both trains. 

... oh and no trains may pass through 42 st because of smoke filling up in All tunnels. (Except 6 Avenue and Lex)

Keep in mind,  (MTA) buses cannot be used in area's of high danger like Coney Island, Jackson Heights, or Times Square.
