Yep.  Here's one that is "normal:"

The switches in the vicinity of the (E) platform at Chambers have gone all FUBAR and will need two weeks to be properly replaced.

There is a crime scene investigation going on on the upper level of 81st Street on the CPW line, requiring all trains use the express track on that level and closing off that station to passengers (southbound trains are unaffected with regard to traveling through the station, but they can't stop there).

Switches at West 4th are such where all trains on the 8th Avenue local tracks must go to the 6th Avenue line in both directions (northbound to 47-50, southbound to Jay Street).  

There is a rail condition on the northbound Broadway Local track inside the 57th Street-7th Avenue station as well as another on the southbound local track in the area of 28th Street.

A train on the Bay Ridge Branch derails and hits the support structure of 62 St-New Utrecht Av. No trains can pass through.

A (G) train stalls just south of Hoyt-Schemerhorn, blocking all tracks except for the <F>

A (R) train derails at Queens Plaza, making no trains able to pass through.

At the same time, Bergen St interlocking also has a meltdown, and no trains cass pass through the area.

  • A (J) train overshoots the track at Broad St, closing off access on the NB (R) track. The (J)(Z) cannot stop at Broad St.
  • An <F> train derails on the 9 St Bridge, causing disruptions to the (F)(G)
  • An (E) train stalls at Canal St, blocking all SB (C)(E) trains,

Reroute (R)(W)(J)(Z)(F)<F>(G)(C)(E).

(F): Runs from Jamaica - 179 St to Bergen St and from Church Ave to Coney Island.

(G): Southern terminus cut to Bergen St.

(R): Runs from Forest Hills - 71 Ave to Canal St (upper level) and from Jay St to Bay Ridge - 95 St.

(W): Suspended.

(Z): Western terminus cut to Chambers St.

A terrorist organisation attempting to attack City Hall detonates explosives in a (6) train on the City Hall loop, causing a cave in of the (4) and (5) tracks above. No trains may pass on either level.

A major signal fault occurs at W 4 St, meaning that no train (with the exception of the 8th Ave express tracks) can be sure of where it will go, and as a result, you are advised to terminate all service through the station.

The first thing I would to do is to shut down the entire system for safety reasons.

But anyway...

and for extra safety, I would send (R) down the Bridge, suspend (W), and terminate (J)(Z) at Chambers or Essex

Next:

This time, a third fight on (MTA) ended up disastrous and one of the executives hit a collection of switches. This shut off power west of Broadway (exclusive) between Houston Street and 57th Street (inclusive).

For a weird reason, the switches south of 59 St and the tracks that (B)(D) use between 59 St and 7 Av has still has power. You can terminate trains at 34 St, there's enough power there.

Also, there is a partial power outage in Seventh Avenue Line between Columbus Circle and Houston Street. You can send trains here, but if you try to send over 10 TPH between Columbus Circle and Houston Street, it loses power. This affects both directions.

Summarized:

West of Broadway shut off, but Broadway is not affected. This only happens between Houston and 57th Streets.

However, the power was still not turned on in the (B)(D) tracks between 59 St and 7 Av.

And lastly, a partial power outage means you cannot run over than 10 TPH between Columbus Circle and Houston Street.

(A) : Trains run local on 8th Av bet 59th & West 4th Sts, then on  (F) line to/from  Jay St.

(C) : Trains run via  (F) between West 4th and Jay Sts.

(E) : Trains rerouted over  (F) between West 4th St and 2nd Av.

8th Av Line shuttle bus will operate betweeen West 4th St &Fulton St IRT/IND Station, stopping at Spring St, Canal St.

Broadway Line Shuttle Bus will operate between Canal St and Wall St, stopping at Chambers St/Brooklyn Bridge-City Hall for Cortlandt St, Fulton St.

(F) : (F) runs via Culver Express track. Trains will be held at Carroll St and Church Av due to single track operation. <F> express suspended.

(G) : Service suspended. Shuttle bus operate between Court Sq and Church Av. B62, B43 will also cross honor, well as NYC Ferry's East River Ferry.

(R) Trains run via Manhattan Bridge between Canal St and DeKalb Av.

(W) : No trains between Canal St & Whitehall St.

(PATH) : Newark-WTC train terminates at Exchange Place. Bus service will be provided between Exchange Pl and Fulton St (2)(3)(4)(5)(A)(C) station at WTC.
