SQL Errors resurfacing: Lost accounts

Looks like the major headaches with IP Board which caused problems with member's accounts back in late March is starting to come back to haunt us. Lance's account is now suddenly inactive and labeled 'Guest'. Just noticed it.

 

I am sifting through posts to see if anyone's else accounts has been affected.

I see a new account has been created for Lance. I didn't realize the issue was already being handled.
