For those selected candidates who, in the foreseeable future, will be reporting to the 5th Floor of 180 Livingston for employment processing for either NYC Transit, MaBSOTA, MTA Bus Company and/or the Staten Island Railway, below is a copy of the Pre-Employment Application Background Verification Questionaire if you need it. In addition, if "Employment Operations" gives you homework, in the form of filling out the Comprehensive Personnel Document Booklet (CPD-B), below is a link to a copy.
