W-4 form

Where/How do get the w-4 form for Federal & state tax deductions. When i did final processing they gave me a paper with my starting salary but under state and federal it says im claiming 0. I dont remember filling out any paperwork for tax withholding or even direct deposit. My training starts in about a week. I want to know how do i update this before i start training and get my first paycheck.
