Track connections between separate lines and divisions

I've always been curious, where are there track connections (other than the obvious Queensboro Plaza) between the A and B divisions, the Flushing Line and the rest of the A division, and between the subway system and other railroads? Does anyone have a list or a map of these?

This connection is not for revenue service due to the differing widths of the trains:
